The Waters of McKenzie is now hiring for a Temporary Social Service position.
Position will last for 3 to 5 months.
Must have LTC experience.
Duties and Responsibilities:
Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needs
Identifying individual social and emotional needs
Developing and maintaining individualized care plans
Participating in the planning of the resident’s admission, community re-entry or transfer to another facility
Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
Work with the finance team to ensure resident is educated and the proper documents are in place for the resident during their stay
Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community
Qualifications/Requirements:
At least one year experience in Social Work field required
Experience in long term care and/or sub-acute setting preferred
