* Has a valid Nurse Aide State Registration in Kentucky or willingness to complete.

*JOB DUTIES:*
· Performs any combination of following duties in care of residents under direction of nursing and medical staff.
· Answers signal lights, bells, or intercom system to determine residents' needs.
· Bathes, dresses, and undresses residents.
· Serves and collects food trays and feeds residents requiring help.
· Transports residents, using wheelchair or wheeled cart, or assists residents to walk.
· Turns and repositions residents requiring physical assistance, alone or with assistance, to maintain good skin integrity.
· Changes bed linens, runs errands, direct visitors, and answers telephone as needed.
· Takes and records temperature, blood pressure, pulse and respiration rates, and food and fluid intake and output, as directed.
· Cleans, sterilizes stores, prepares, and issues supplies as needed.
· Reports resident status to the nurse on duty and documents in the written medical record as required.
· May be assigned other duties.
