Chronic Care Management (LPN or MA)

We are seeking an LPN or Certified MA Care Coordinator to develop personalized care plans and perform telephonic encounters with patients on behalf of our providers.
Manage and support patient's healthcare needs via phone conversations Prepare patients for appointments and procedures, and provide follow-up as needed Requires Active LPN LIcense or MA Certification Experience in care coordination or care management Hands on experience with Electronic Medical Records Strong Computer skills required Job Type: Full-time Pay: From $14.
