MAIN FUNCTION:
The Medical Department Coordinator job profile encompasses a multitude of tasks that contribute to the smooth running of Nia Association’s health care operations.
The Medical Department Coordinator will work alongside the Director of Services and the Agency Nurse to ensure that quality medical care is afforded to our Persons Supported.
The Medical Department Coordinator will be responsible to coordinate with the Agency Nurse to ensure that all appointments and calendars are incongruent.
The Medical Department Coordinator must have the knowledge, values and skills to perform in an exemplary manner as well as the ability to work independently.
This duty may encompass the areas listed below and other relevant medical duties as assigned.
DUTIES Administrative Maintain a monthly calendar that depicts appointments for the individuals supported by this Agency and ensure that Program Coordinators and other staff are kept abreast of all appointments and scheduled changes.
Ensure all Nursing Services documents and Monthly Nursing Summaries are scanned into Medical Server, in individual folders for each Person Supported.
Use Therap program to place and manage medical information for every Nursing Services client.
Review for each Person Supported and assist Agency Nurse with Medication Administration Report (MAR), prescriptions and medication profile prior to release to homes (per Agency and DIDD policy and guidelines.
) Work with the Agency Nurse to review medical profile sheets when medication changes occur to ensure there are no duplicate/triplicate scripts for the same diagnosis or contraindications by different physicians.
Fax new prescriptions or refill orders to designated pharmacy in a timely manner.
Obtain lab results, vitals and other medical procedures per DIDD and Nia standard.
Acquaint Agency Nurse with all actions undertaken as it relates to prescriptions, refills, labs, vitals, and other medical information.
Conduct quarterly audits of the First Aid Kits and replenish supplies for distribution per Agency policy.
Dialogue with direct care staff as it relates to the medication passes and the seven rights in order to ensure compliance with the administration of medication.
Coordinate with the Agency Nurse or certified staff the disposal of medication and maintenance of controlled substance (per State guideline and licensure regulation).
Work alongside the Registered Nurse to ensure that Domain 5 is upheld and the Agency meets medical mandates and directives per State and licensure protocols.
Direct Care Assist the Director of Services as needed.
Obtain and record vital signs and weight on a monthly basis for our Persons Supported.
Assess the situation in the absence of the Agency Nurse and notify RN or MD as deemed necessary (follow protocol for emergency crisis).
Confer with physicians as needed in regard to protocols or treatment plans and relay information to the Registered Nurse.
Report significant weight gain/loss as well as vitals and other concerns to the Agency Nurse, Practitioners as well as the Executive Management Team in a timely manner.
Medication Pickup Ensure that medications are picked up from pharmacy when ready.
Communicate with Program Coordinator(s) as it relates to medication being available.
Perform mandatory checks prior to departing the pharmacy as well as handing over medication to Program Coordinator/Home Director.
Distribute medication to Home Directors during the last week of the month.
Ancillary Duties Present medical information as it relates to our Persons Supported during regularly scheduled Leadership meetings.
Provide documentation to ISCs, BAs and others as needed for records completion.
Assist in packaging information as needed for the Human Rights Committee.
Serve as a valuable member of various committees as needed (i.
e.
HRR, IMC, etc).
Visit homes as assigned as it relates to quality inspections NOTE: The positions at Nia Association are driven by the needs of our Persons Supported.
Shortages, administrative holds, and other noted changes may require all internal staff to assist in other capacities outside their normal day-to-day assignment.
Requirements: Requirements Training/Experience: 1.
High school diploma / College preferred, but, relevant experience acceptable.
2.
LPN license is ideal, but not required.
The LPN would assist the Agency with other medical responsibilities.
3.
Must have a valid drivers’ license, with driving record acceptable to insurance carrier.
3.
Experience working with individuals who have developmental disabilities preferred, but not required.
5.
Prefer two or more years of experience in physician’s office setting with emphasis on managing appointments and medical charts but willing to train right person.
Knowledge, Skills and Abilities: 1.
Proven ability to multitask.

Possess excellent communication skills.
3.
Able to manage records and conduct extensive follow up.
4.
Ability to safely lift 25-45 lbs.
with/without assistance.
