Job Description
WHAT YOU WILL DO:
The Case Manager oversees and monitors the progress of individuals on an assigned caseload.
The Case Manager is responsible for ensuring staff is providing services in accordance with the person’s served plan for treatment.
Position requires travel
RESPONSIBILITIES AND DUTIES
Attends industry functions to promote positive relationships with other providers.
Develop positive relationships with all funding sources to promote new business.
Ensures the overall implementation and practice of infection control procedures.
Coordinates the scheduling of team meetings (times, dates, locations, etc.
) and informs all team members of such.
Develop and implement a service plan within 30 days of moving in, annually, when significant changes occur, and when moving out.
Ensures consumer and guardian participation in the development of service and personal futures plans.
Coordinates development of each person(s) served personal futures plan.
Schedules and chairs screening for vacancies.
Ensure all written training programs are implemented and revised as needed.
Monitors/implements/supervises delivery of service plans and personal future plans and training of staff.
Documents consumer progress or regression.
Initiates discharge planning if appropriate.
Maintains records appropriately.
Serves on assigned committees.
In-services staff as requested.
Qualifications
WHAT WE LOOK FOR:
Bachelor's degree in Social Work or related field preferred
Minimum two years’ experience in the field of Intellectual or Developmental Disabilities preferred
Must be organized and have great communication skills
Additional Information
HOURS:
The Case Manager’s schedule must be flexible to accommodate various program, individual, and staff needs.
Office hours are typically 8:00 am – 5:00 pm, Monday-Friday.
Events such as staff and individual meetings, advisory board meetings, etc.
, do not always fall within the usual hours, but Case Managers are expected to attend.
